Partnering with Regional Centers to Support Individual Success

At ICARE Support Living Services, we are committed to working closely with Regional Centers, Service Coordinators, individuals, and their support teams to ensure timely, person-centered services that promote independence, self-determination, and community inclusion.

We understand the important role Service Coordinators play in connecting individuals to the supports they need. Our team strives to make the referral process simple, secure, and efficient while maintaining open communication throughout the intake and assessment process.

How to Submit a Referral

To begin the referral process, please upload the individual's documents through our secure and encrypted referral portal.

We encourage Service Coordinators to submit the following documents, when available:

  • Individual Program Plan (IPP)

  • Client Development Evaluation Report (CDER)

  • Other related documents

Submitting complete documentation helps our team review the referral promptly and determine the most appropriate supports and services for the individual.

Our Commitment

Upon receiving a referral, ICARE Support Living Services will:

  • Review all submitted documentation

  • Contact the Service Coordinator regarding any additional information needed

  • Schedule assessments and intake meetings as appropriate

  • Collaborate with the Regional Center and interdisciplinary team

  • Develop individualized service recommendations based on the person's goals, strengths, and needs
